Jordan Brignac, MSN, APRN, FNP-C
Jordan Brignac, MSN, APRN, FNP-C, is a compassionate and dedicated Family Nurse Practitioner with over 14 years of experience in healthcare, including five years as a registered nurse and nine years as a board-certified nurse practitioner. Raised in Ascension Parish, Louisiana, Jordan earned her Bachelor of Science in Nursing from Our Lady of the Lake College and her Master of Science in Nursing, Family Nurse Practitioner, from LSU Health Science Center. Her extensive clinical experience includes primary care, preventative medicine, and integrative wellness, giving her a comprehensive perspective on patient-centered care.
In September 2025, Jordan co-founded Revolution Wellness, a direct primary care practice in Prairieville, Louisiana, with her business partner Holly Valentine. The practice is membership-based and emphasizes relationship-centered care, allowing longer appointment times for meaningful conversations, comprehensive evaluations, and preventative health strategies. Jordan is particularly passionate about addressing chronic conditions prevalent in her community, such as obesity and diabetes, through education, individualized care, and long-term trust-building with her patients.
Jordan’s approach to medicine is informed by early mentorship, during which she observed a seasoned physician for several months, learning the value of listening and cultivating lasting patient relationships. This philosophy, summarized in her mantra “build relationships, not bills,” guides every aspect of her practice. Outside of work, Jordan enjoys spending time with her two children, attending LSU sporting events, and prioritizing family activities. Her commitment to holistic, patient-focused care continues to make her a trusted provider in the Baton Rouge Metropolitan Area.

What do you enjoy most about practicing medicine?
What I value most is the opportunity to build meaningful relationships with my patients and provide them with the time and attention they truly deserve. Rather than moving quickly through brief, time-constrained visits, I’m now able to spend 30 to 60 minutes with each patient, allowing for thoughtful conversations and a deeper understanding of their health needs. This approach enables me to deliver truly comprehensive and preventative care, focusing not only on treatment but also on long-term solutions that improve overall well-being. I am passionate about listening to my patients and ensuring they feel informed and empowered in their healthcare journey. In our community, where conditions such as obesity and diabetes are prevalent, education plays a critical role in improving outcomes. Taking the time to explain, guide, and support patients has helped foster strong trust — something that was deeply instilled in me by the physician who mentored my training. It is incredibly rewarding to see people meet their health goals. I love being a part of that journey with them, it reinforces a simple truth: meaningful care begins with listening.
