Kimberly Diaz, RN, is the founder of Care Matters LLC and Caring Minds Dementia Consulting and Training. With over 26 years of nursing experience, she specializes in personalized care solutions for seniors and their families, focusing on dementia care, aging transitions, and end-of-life services. Through Care Matters LLC, Kimberly connects families with qualified independent contractors, providing advocacy, care navigation, relocation and placement support, private duty nursing, and end-of-life guidance.

At Caring Minds Dementia Consulting and Training, she offers expert guidance, education, and training for family caregivers and healthcare professionals, helping them navigate the complexities of dementia and aging with clarity, compassion, and confidence. Her trauma-informed approach supports adult children who may be facing unresolved personal challenges while caring for a parent.

Kimberly’s mission is to ensure every family receives the support they need, empowering caregivers and enhancing the quality of life for seniors. She is committed to setting the standard in senior care, advocating for dignity, respect, and individualized solutions for every client she serves.
