Kimberly Diaz, RN, is a seasoned nursing professional and the founder of Care Matters LLC, Care Matters Registry, LLC, and Caring Minds Dementia Consulting and Training. With over 26 years of nursing experience, she specializes in personalized care solutions for seniors and their families, with a strong focus on dementia care, aging transitions, and end-of-life support. Her work is driven by a deep commitment to dignity, compassion, and individualized care for every client she serves.
Through Care Matters Registry, LLC, Kimberly connects families with qualified independent contractors who provide personalized, affordable care tailored to each family’s needs. Care Matters, LLC offers comprehensive services including advocacy, care navigation, relocation and placement support, private duty nursing, and end-of-life guidance. Together, these services ensure families receive coordinated support during some of life’s most challenging transitions.
At Caring Minds Dementia Consulting and Training, Kimberly provides expert education, consultation, and training for both family caregivers and healthcare professionals. Her trauma-informed approach is especially supportive of adult children caring for aging parents, recognizing the emotional complexities and unresolved challenges that often arise during this journey. She empowers caregivers with clarity, confidence, and practical tools to navigate dementia and aging with compassion.

What do you enjoy most about practicing medicine?
It’s the human connection.
It’s sitting with someone in one of the most vulnerable seasons of their life and helping them feel seen, heard, and safe. It’s translating fear into understanding, confusion into clarity, and overwhelm into “okay… I can do this.”
In dementia care especially, medicine isn’t just about treatment — it’s about presence. It’s about honoring someone’s story, advocating when their voice is fading, and guiding families through the question, “Where do we go from here?”
I love being the steady hand in the uncertainty.
