Karen Claise is a seasoned Nurse Practitioner Consultant specializing in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM), serving patients and healthcare organizations throughout Greater Philadelphia and New Jersey. With nearly two decades of experience as a nurse practitioner and a career that began in critical care nursing, Karen brings a rare blend of clinical depth and operational insight to her work. Her expertise spans geriatric and nursing home medicine, psychiatric medication management, primary care, urgent care, and correctional health. She has played an integral role in implementing and leading CCM and RPM programs, helping practices improve outcomes, enhance care coordination, and reduce unnecessary hospitalizations.

Karen’s professional background includes extensive leadership and management experience, including long-standing service with Duke University Health System and executive clinical roles in care management organizations. Her foundation in cardiology, open-heart and neuro critical care, respiratory intensive care, and congestive heart failure management shaped her strong clinical judgment and systems-thinking approach. In addition to direct patient care, she has contributed to quality improvement initiatives, EMR implementation, data-driven care models, and interdisciplinary team development. She is passionate about reducing inappropriate psychotropic use in vulnerable populations and advancing holistic, patient-centered models of care that address root causes—not just symptoms.

A graduate of Fairleigh Dickinson University (BSN) and the University of Southern Indiana (Master’s education), Karen views healthcare not simply as a profession, but as a calling. She often describes her work as a ministry rooted in compassion, advocacy, and service. Active in community and faith-based nursing initiatives, she has dedicated decades to supporting patient education and wellness beyond the clinical setting. Karen advises the next generation of nurses and nurse practitioners to seek strong mentorship, cultivate critical thinking, and never lose sight of the patient and family as the true focus of healthcare.

What do you enjoy most about practicing medicine?

What I enjoy most about practicing medicine is the opportunity to provide meaningful, patient-centered care—particularly to vulnerable populations in long-term care and psychiatric settings—while building relationships that truly impact outcomes. Although the field faces significant challenges, including corporate and insurance pressures, declining reimbursement, extensive documentation requirements, staffing and operational constraints in long-term care, and increasing demands to see more patients at the expense of quality, I remain focused on delivering compassionate, thoughtful care. At the same time, I see tremendous opportunity in the growing demand for nurse practitioners in nursing homes and psychiatric roles, as well as in the expansion of telehealth, chronic care management, and remote patient monitoring programs, all of which allow us to improve access, coordination, and overall quality of care.
