Dr. Iwalani “Iwa” Crush, PT, DPT, is a certified pelvic rehabilitation practitioner with over 16 years of experience helping clients achieve bodily autonomy through pelvic education and strengthened mind–body communication. She is the founder of Pelvic Guide LLC in Springfield, Pennsylvania, where she provides trauma-informed, integrative care for women, men, and individuals undergoing gender affirmation. Her practice focuses on pelvic pain, sexual dysfunction, incontinence, and chronic pelvic conditions, blending pelvic floor physical therapy, sex counseling, and women’s health coaching to empower patients and support evidence-based healing.
Born in Hawaii and professionally shaped in New York City, Dr. Crush earned both her undergraduate degree and Doctor of Physical Therapy from New York University in 2009. Her passion for physical therapy began at age 15 while volunteering at Shriners Hospital for Children, where she discovered the profound impact of human connection, rehabilitative care, and lifelong learning. Since then, she has trained and worked with prestigious institutions including NYU Hospital, Columbia Presbyterian Hospital, Hospital for Special Surgery, Weill Cornell Medical System, Mount Sinai, and Burke Rehabilitation Hospital.
In addition to running her private practice, Dr. Crush serves per diem with Penn Medicine/Good Shepherd Penn Partners, teaching in their pelvic health residency program, and contributes to physical therapy education through Arcadia University and the Herman & Wallace Pelvic Rehabilitation Institute. Her approach combines evidence-based Western medicine with holistic frameworks, integrating bio-psychosocial perspectives to address both the physical and emotional dimensions of pelvic health. She credits her neurodivergent lens and lifelong curiosity as key influences on her innovative, patient-centered care.
Inspired by her Hawaiian namesake, the “Iwa” (Great Frigate Bird), Dr. Crush sees her role as guiding patients through complex physical and emotional landscapes, fostering connection, safety, and empowerment. Known for her clinical precision and expansive curiosity, she is committed to helping patients navigate their healing journeys while fostering education and embodiment. Her personal philosophy emphasizes holistic care, advocacy, and compassion, bridging science with a human-centered approach to pelvic health.
Outside of her professional work, Dr. Crush values family, relationships, and her animal companions—her Frenchie, Face, and her cats, Cathexis and Ramses. She celebrates her roots and cultural heritage while enjoying life with her partner and deep family connections. What do you enjoy most about practicing medicine?
What I enjoy most is being able to piece together all of a patient's history and information to help them understand what's going on with their body, especially when they've been struggling for years trying to figure it out. I love being able to affirm everything they're going through and validate that what they're experiencing is real. So many of my clients tell me they've been to so many doctors for years, and when I can show them basic pelvic floor anatomy and physiology and how it connects to different systems, they often say 'why did my doctor never tell me this?' or 'I had no idea' or 'oh my god, this is why I'm experiencing this symptom.' Sometimes their doctors have told them it's all psychological or in their head, and I can help them understand that yes, it may be in their head, but not the way they were told - especially with chronic pain conditions. I also really enjoy the human interactions, the ability to have humor and empathy and joy even in serious or sad situations, and being able to make a real difference in people's lives when they may have lost hope.
