Dr. Jonathan M. Collins, MD, FAAFP

Dr. Jonathan M. Collins, MD, FAAFP, is the Chief Strategy Officer at One Health Value Solutions, where he leverages over two decades of experience in physician leadership to transform healthcare delivery. A board-certified Family Physician, Dr. Collins has practiced in Charlotte, North Carolina, providing full-time primary care while leading strategic growth initiatives, care team design, and provider wellness programs across multiple clinical settings. He is deeply committed to improving patient-centered care, guiding hundreds of primary care providers through value-based care transitions, compensation plan enhancements, and the development of dyad leadership models that ensure accountability and high-quality outcomes at the local level.

Throughout his career, Dr. Collins has focused on reducing physician burnout and fostering sustainable, fulfilling careers for healthcare providers. He has developed onboarding and mentorship programs that integrate mindfulness, professional development, and education to support clinicians in delivering exceptional care while maintaining their well-being. His work at Modern Primary Care Consultants aims to reshape the healthcare landscape by creating an environment where patients receive affordable, accessible, and high-quality care while keeping the provider-patient relationship central to the model.

Dr. Collins earned his Bachelor of Science in Biology/Pre-Med from North Carolina State University and his MD from the Brody School of Medicine at East Carolina University, where he was an Albert Schweitzer Fellow and served as chief resident during his residency at Atrium Health Carolinas Medical Center. He and his wife, Shannon, are proud parents of seven children, including through adoption, and one grandson. Outside of his professional endeavors, Dr. Collins is devoted to his faith, family, and mentoring the next generation of healthcare leaders.
