Jennifer Fortin, LPN, is a dedicated and highly skilled complex care nurse specializing in adult acute care, with expertise in tracheostomy and ventilator-dependent patients. She currently provides full-time care for a patient in Culpeper, VA, diagnosed with Duchenne’s Muscular Dystrophy. Known for her compassion and meticulous attention to detail, Jennifer ensures her patients thrive in their home environments, earning the trust and admiration of both patients and their families.
Before joining Hi-Pointe in 2016, Jennifer gained extensive experience across multiple healthcare settings, including nursing homes, long-term care facilities, rehabilitation centers, and hospital ICU units. Raised in a family deeply committed to nursing—her parents own a nursing home, and her sisters are also nurses—she embraced nursing as her calling. Jennifer was drawn to home care for its opportunity to work closely with patients, monitor their progress, and provide personalized, ongoing support.
Jennifer takes pride in being part of Hi-Pointe, where she is challenged to meet high standards and benefits from continuous training and mentoring. Beyond her professional dedication, she actively supports the VA and Paralyzed Veterans, reflecting her commitment to service. Outside of work, Jennifer enjoys spending time with animals, combining her compassion for people and pets alike.
