Tristan West, MSN-Ed, RN, is an Assessment Manager and hospice care consultant with nearly five years of nursing experience spanning acute cardiac care, PCU, home health, and hospice. She currently oversees multiple sites in a consultant capacity, focusing on hospice education and clinical support. Tristan holds a Master of Science in Nursing Education from Liberty University and brings a strong foundation in both patient care and academic instruction to her work.
Before transitioning fully into hospice, Tristan served as a nursing instructor, where she taught health assessment and guided students through clinical practice. Her passion for education remains central to her role today, where she equips nurses with the knowledge and confidence to better understand hospice services and deliver compassionate, patient-centered care. She is deeply committed to breaking down barriers to hospice access and improving outcomes for patients and their families.
Driven by a desire to make a broader impact, Tristan focuses on mentoring and developing nurses across the teams she supports. Through triaging, chart audits, and real-time coaching, she empowers clinicians to navigate challenges and provide high-quality care.
