Susie Schmaltz, AGACNP-BC, CCRN

Susie Schmaltz, AGACNP-BC, CCRN is a highly driven, board-certified Adult-Gerontology Acute Care Nurse Practitioner with a specialized focus in cardiology and advanced heart failure management. With more than 15 years of direct patient care experience, she brings a deep clinical background spanning critical care, electrophysiology, and complex cardiovascular disease, including LVAD and transplant care. She is currently a Heart Failure Program Manager and Lead Nurse Practitioner at Circadian Health, where she supports value-based care delivery through multistate-licensed virtual telemedicine services. Following graduate training, Susie was selected for a highly competitive Cardiology Nurse Practitioner Fellowship at Mayo Clinic Arizona—an ANCC-accredited program accepting only two fellows annually. This intensive fellowship provided advanced, mentored training across the full spectrum of adult cardiovascular care, including advanced heart disease, transplantation, electrophysiology, interventional cardiology, and cardiac surgery.

Prior to her current role, she held leadership and clinical positions with HonorHealth and the Mayo Clinic, managing congestive heart failure clinics and caring for patients in both inpatient and outpatient cardiology settings. Passionate about evidence-based practice and patient empowerment, Susie is recognized for her strong communication skills, collaborative interdisciplinary approach, and ability to manage complex cardiovascular conditions with precision and compassion. Her professional mission centers on improving outcomes for cardiac patients through education, innovation, and high-quality, patient-centered care.
