Dr. Sheeva Chopra is a Clinical Pharmacist specializing in ambulatory care, currently practicing with Included Health. She is dedicated to chronic disease state management and medication therapy management, providing evidence-based, patient-centered care. Dr. Chopra earned her Doctor of Pharmacy (Pharm.D.) from the University of Oklahoma Health Sciences Center and completed a PGY-1 Pharmacy Practice residency at Parkland Health & Hospital Systems, followed by a PGY-2 Ambulatory Care residency at The Johns Hopkins Hospital.
Throughout her career, Dr. Chopra has focused on optimizing medication use, educating patients and healthcare teams, and advancing pharmacy practice through research and preceptorship. As a Clinical Pharmacy Specialist, she evaluates medication processes, delivers practice-based education, and precepts students to ensure high-quality care. She holds active state licensures in Texas, Massachusetts, Washington, and Florida (for telehealth), Arkansas, Louisiana, Michigan, New Hampshire, New Jersey, Oregon, Utah, and Washington.
Beyond her clinical work, Dr. Chopra is passionate about improving patient outcomes through collaborative care and innovation in pharmacy practice.
