Penny Simoes is a dedicated healthcare professional serving as the Director of Discharge Planning and Case Manager at Care One Rehab. With extensive experience in nursing and patient care, she plays a critical role in coordinating comprehensive discharge plans, ensuring patients receive the support and resources they need for a smooth transition from rehabilitation to home or other care settings. Penny began her nursing career after earning her RN from Signature Healthcare Brockton Hospital School of Nursing in 2008. Over the years, she has gained a wealth of experience across various healthcare settings, including home health as an RN case manager, and acute care as a visiting nurse. She also served as a Registered Nurse with Encompass Health, further broadening her expertise in patient care and case management. Her professional journey reflects her dedication to supporting patients through every stage of recovery.

Outside of her current role, Penny is known for her ability to lead interdisciplinary teams with compassion and efficiency. She values continuous learning and applies her extensive clinical knowledge to mentor colleagues and improve care delivery systems.
