Nazeneem Kleiman, FNP-BC
Nazeneem “Naz” Kleiman, FNP-BC, is a board-certified Family Nurse Practitioner with over 17 years of nursing experience and a distinguished career dedicated to patient-centered, evidence-based care. She is the Founder and Medical Director of Kleiman Healthcare Services, where she delivers comprehensive primary care with a strong emphasis on trust, compassion, and clinical excellence. Her work reflects a modern vision of medicine—one that blends rigorous clinical standards with a deeply personalized approach to healing.
Naz earned her Bachelor of Science in Nursing from the University of Texas and later completed her Master of Science in Nursing at Walden University. Her academic foundation, combined with years of hands-on clinical practice, prepared her for a dynamic career spanning bedside nursing, advanced practice, and clinical leadership. She is recognized for her ability to integrate diagnostic precision with thoughtful, relationship-driven care.
Her professional background includes extensive training and practice within major hospital systems in Houston and Portland, where she gained invaluable experience in high-acuity and medically complex settings. These environments strengthened her expertise in primary care, trauma and urgent care, chronic disease management, and women’s health. Over time, Naz made the intentional decision to step away from traditional healthcare systems to build a practice that allows for greater autonomy, deeper patient relationships, and a more meaningful care experience.
Naz’s clinical approach centers on attentive listening, collaboration, and individualized treatment planning. She believes optimal outcomes are achieved when patients feel heard, respected, and empowered to actively participate in their healthcare decisions. Her areas of specialization include preventive care, women’s health, fertility support, medical weight loss management, hormone therapy for women and men, and both acute and chronic condition management.
Her work is informed not only by clinical expertise, but also by lived experience. As a mother of two teenagers conceived in her 40s, Naz brings a deep personal understanding of the emotional and physiological complexities surrounding fertility, hormonal transitions, and life-stage wellness. This perspective allows her to connect with patients on a deeper level and to deliver care that is both clinically sound and emotionally attuned.
As the only African American woman–owned healthcare practice in her area, Naz leads with a strong sense of purpose and responsibility. She is committed to advancing excellence, representation, and innovation in healthcare, while also mentoring future professionals through preceptorship and academic collaboration. What do you enjoy most about practicing medicine?
I enjoy forming long-term relationships with my patients and guiding them through every stage of health. Practicing medicine that is thoughtful, evidence-based, and collaborative—where patients feel informed and supported—is what makes this work meaningful to me. Providing care that blends clinical excellence with compassion and trust is both my purpose and my greatest reward.
