Michele Hawthorne, LPN

Michele is a dedicated Licensed Practical Nurse with nearly two decades of experience providing compassionate, patient-centered care. Licensed since 2007, she has built a diverse nursing background rooted in strong clinical skills, adaptability, and a deep commitment to listening to and understanding her patients. Her approach emphasizes trust, communication, and meeting individuals where they are in their healthcare journey.

Currently, Michele works remotely with UnitedHealthcare (Optum), supporting a house-call nurse practitioner program that focuses on annual in-home assessments. In this role, she assists with clinical documentation, screenings, and coordination of care to ensure patients’ needs are accurately communicated to their primary care providers. In addition, she continues to work part-time in a skilled nursing facility, where she has been employed since 2015, maintaining hands-on clinical experience and strong connections with patients and care teams.

Michele’s career path includes experience in hospital rehabilitation settings, skilled nursing facilities, and group home care, giving her a well-rounded perspective on patient needs across care environments. She attributes her success to her ability to truly listen—to patients’ concerns, histories, and lived experiences—which allows her to provide thoughtful, effective support. Passionate about continued growth, Michele remains focused on advancing her skills while delivering consistent, empathetic care.
