Melissa Seccareccio, MSN, RN-BC, is the Executive Director of Care Coordination and Population Health at Southern New Hampshire Health in Nashua, New Hampshire. With more than 26 years of nursing experience, she has built a distinguished career focused on improving patient outcomes through innovative care coordination, quality improvement, and value-based healthcare delivery. Known for her collaborative leadership style, Melissa works closely with interprofessional teams to strengthen care transitions, enhance population health strategies, and ensure patients receive comprehensive, patient-centered care across the healthcare continuum.
Melissa began her career in inpatient medical-surgical nursing before expanding her expertise into post-acute home care and healthcare leadership roles. Over the years, she has overseen hospital medicine and pediatric programs, managed case management initiatives, served as Associate Vice President of Quality, and supervised ambulatory nursing teams within physician practices. Her professional journey reflects a steady progression of leadership and clinical excellence, supported by her academic achievements, including a Master of Science in Nursing with a focus in Care Coordination from Capella University, where she graduated with distinction.
Deeply passionate about serving older adults and individuals with complex health needs, Melissa is an advocate for age-friendly healthcare and sustainable care models that benefit both patients and healthcare professionals. She has been actively involved in advancing age-friendly initiatives within her health system and continues to champion strategies that improve care experiences while supporting the workforce delivering that care. Currently pursuing a Doctor of Nursing Practice at Capella University, Melissa remains committed to transforming healthcare delivery and building healthier communities through compassionate, coordinated, and forward-thinking leadership.
