Jennifer Case, RN, is a highly experienced registered nurse with over 20 years of practice spanning dialysis, disease management, home health, and crisis/psychiatric care. Her career has taken her from traditional bedside nursing to leadership roles, including Director of Nursing, Clinical Supervisor, and Disease Manager. Jennifer has extensive experience working with diverse patient populations, including military personnel, retirees, and pediatric patients, and she has honed her skills in chronic disease management, patient education, and navigating complex healthcare systems.

Throughout her career, Jennifer has demonstrated a deep commitment to building trusting relationships with her patients. While practicing dialysis, she spent several hours with patients multiple times per week, fostering a unique level of care and understanding. In disease management, she bridged gaps between patients, physicians, and payers, helping patients navigate TRICARE, Medicaid, and Medicare while ensuring adherence to treatment plans. Her background in education, with a Bachelor of Science degree in Education and an Associate Degree in Nursing, has enabled her to tailor patient teaching and communication to meet each individual’s needs.

What do you enjoy most about practicing medicine?

What I enjoy most about nursing is building relationships with my patients and having them trust me. When you work in dialysis, you're with patients for 4 to 5 hours, 3 to 4 days a week, so you really get to know them well. It builds a different level of trust than what you have at normal bedside, and I fell in love with that part. I also always value the education that I can provide. Everyone learns differently, and you have to find different ways to teach patients, using terms they understand rather than overwhelming them with medical jargon. Being able to bridge that gap between physicians and patients, helping them understand the disease process and get the resources they need to succeed, is very satisfying to me.
