JaNelle began her career as a Licensed Practical Nurse, gaining extensive experience in skilled nursing, rehabilitation, long-term care, and hospice environments. Her strong clinical foundation led her to expand her impact in community-based and acute care case management roles, where she excelled in guiding patients and families through transitions of care, advocating for essential resources, and reducing avoidable hospitalizations.
Motivated to advance her career and deepen her clinical expertise, she completed her Registered Nursing education at Pickaway-Ross Career & Technology Center in 2024. Today, she serves as a Registered Nurse Clinical Care Coordinator at PrimaryOne Health, where she leads comprehensive care planning, facilitates multidisciplinary collaboration, and supports patient independence through education and tailored interventions. She previously worked as a Case Management Nurse with Mount Carmel Health System and a Population Health Specialist, managing Medicaid members with a focus on preventive care, chronic disease support, and resource navigation.

What I enjoy most about practicing nursing is the opportunity to make a meaningful difference in people’s lives every day. Nursing allows me to connect with patients and families during some of their most vulnerable moments, offering compassion, education, and support that truly impact their well-being. I find fulfillment in advocating for patients’ needs, collaborating with interdisciplinary teams, and continuously learning to provide the highest quality of care. The ability to combine clinical skill with empathy and human connection is what makes nursing not just a profession, but a calling I’m deeply proud to be part of.
