Heather A. Pfeifer, FNP-BC
Heather is a dedicated Family Nurse Practitioner whose career reflects a strong commitment to comprehensive, patient-centered care across multiple healthcare settings. She currently practices in complex care management and hospice services, where she conducts independent health assessments, identifies gaps in care, and delivers thorough evaluations that address medical, functional, psychosocial, and safety needs. Her work emphasizes prevention, education, and coordination of care, ensuring patients receive appropriate follow-up, resources, and support within their communities.
With a clinical background that spans acute care, cardiology, hospice, and care management, Heather brings depth and versatility to her practice. She has extensive experience managing complex cardiac patients in hospital settings, as well as supporting individuals and families through end-of-life care. Earlier in her career, she served in hospice leadership as Director of Clinical Operations, where she led interdisciplinary teams, ensured regulatory compliance, and advanced quality improvement initiatives. This leadership experience continues to inform her collaborative, systems-focused approach to patient care.
Heather earned her Master of Science in Nursing from Walden University, building upon prior nursing education and years of hands-on clinical experience. She is a recipient of the DAISY Award, recognizing her compassion, professionalism, and dedication to excellence in nursing.
