Cyreeta Smith, MSW
Cyreeta Smith, MSW, is a dedicated Member Engagement Specialist and social worker based in Sacramento, California. She currently works for Kaiser Permanente’s Medicaid (Medi-Cal) program, where she connects members to vital healthcare services, coordinates care, and ensures patients have access to the resources they need to thrive. With a strong background in patient advocacy, Cyreeta brings both compassion and clinical expertise to her work supporting diverse populations across the region.
Cyreeta’s professional focus centers on improving access to care for Medicaid members through targeted outreach and comprehensive care coordination. She conducts chart reviews to identify unmet or chronic needs, facilitates connections to in-house and community-based resources, and serves as a liaison between members, healthcare providers, and social service agencies. Her work includes coordinating care plans, arranging transportation, and addressing social determinants of health to ensure members receive timely, appropriate, and equitable care.
Her career in healthcare and social work spans hands-on experience in hospice care, bereavement counseling, community mental health, and case management. Cyreeta has collaborated closely with interdisciplinary teams including physicians, nurses, and fellow social workers to support patients and families navigating complex life transitions, including end-of-life care. She has also contributed to community-based initiatives focused on health equity, trauma-informed practice, and culturally responsive services.
Cyreeta earned her Bachelor of Arts in Sociology from San Francisco State University and has complete her Master of Social Work in Clinical/Medical Social Work from Grand Canyon University in October 2025. She is an active member of the National Association of Social Workers and remains committed to lifelong learning, professional development, and service excellence in both healthcare and community settings.
Outside of her professional work, Cyreeta enjoys creative and outdoor pursuits. She has a passion for art and fashion, expressing her personal style through curated outfits, as well as nature photography and gardening. Through both her professional and personal endeavors, Cyreeta demonstrates a deep commitment to fostering well-being, equity, and growth in the communities she serves.
