Crystal Farrow is a dedicated registered nurse with 14 years of experience specializing in care management, care coordination, and transitions of care. Currently serving as a transition coordinator with CenterWell Senior Primary Care
, Crystal works closely with patients following hospital discharge to ensure continuity of care and successful recovery at home. Her responsibilities include coordinating post-discharge services such as home health care, subacute rehabilitation, and durable medical equipment, while also educating patients on disease management, medications, side effects, and when to seek emergency care. Passionate about reducing hospital readmissions, Crystal believes patient understanding and empowerment are essential to long-term health outcomes.

Crystal earned her associate degree from Albany State University, followed by a Bachelor of Science in Nursing from Capella University and a Master of Science in Nursing from Walden University in 2025. She is a nationally certified case manager and holds train-the-trainer certification, reflecting her commitment to professional excellence and patient advocacy. Continuing to expand her expertise, Crystal is currently pursuing a second master’s degree to become a nurse practitioner, with plans to complete the program in 2027. She is also an active member of the American Nurses Association and the National Society of Leadership and Success.

In addition to her clinical work, Crystal has participated in public health education initiatives and community outreach. In 2024, she appeared in a 30-minute segment on WSB-TV discussing diabetes and hypertension, and also contributed to a National HIV Day segment focused on awareness and education. Inspired by her mother, a surgical technician, Crystal developed an early interest in healthcare through technical courses in high school, where she discovered her passion for nursing, research, and problem-solving. Today, she remains committed to helping patients better understand their health conditions and empowering them with the tools and confidence needed to manage their care successfully at home.

What do you enjoy most about practicing medicine?

I enjoy seeing the victories with my patients. Even with getting my master's, I still touch with my patients, but I also train my staff as well. Those victories with my staff are important too, because education in this particular realm is such a big thing. Knowing that you get the victories by giving the education, both with the patients and with my staff, and seeing those small victories and knowing that I helped get there, that is such a fulfillment for me.
